Insurance Accepted
We accept self pay and Major Insurances such as United Health Care, Aetna, Blue Cross Blue Shield, Cigna, and hundreds other insurances.
How To Pay
We have a sliding scale from depending on annual household income, before taxes. Proof of income required for sliding scale. We can see you the day you call or whenever possible. We have daytime, evening and weekend appointments available.
We require a Visa or Master Card to set an appointment. We have a $50 cancellation, reschedule policy. We only charge the cancellation, reschedule fee if you cancel, re schedule or do not show up.
